Hello from Elite Mental Health & Wellness, LLC. 

My name is Rhonda Dolen-Hooker, and I would like to thank you for choosing Elite Mental Health & Wellness, LLC, for your or your loved one’s mental health care services. 

I am a board-certified family and psychiatric-mental health nurse practitioner (FNP-BC and PMHNP-BC respectively). My collaborating physician is Dr. Jennifer Powell, a family medicine physician, in Osage Beach and Lebanon, Missouri. 

My goal is to provide you with exceptional mental health care services to assist you in reaching your optimal level of mental health and well-being.  I provide psychopharmacology (medication management), psychotherapy services, counseling, and overall holistic health education for a diverse population. I offer in-person and telehealth services for the convenience of my clients. I am trained in Accelerated Resolution Therapy (ART), a remarkable eye movement therapy, developed in 2008, that provides accelerated consistent and reliable relief from symptoms related to underlying trauma(s), addictions, phobias/fears, anxiety, depression, and many other disorders/conditions. In addition, I am trained in Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and other types of psychotherapy.

Some Practice Guidelines I would like to share with you are as follows:

  • Normal business hours are 8:00 a.m. to 4:30 p.m. Tuesday through Thursday and 8:00 a.m. to 2:00 p.m. Friday.
  • You can reach me by phone during the day at the following phone numbers:

  • Unless it is after hours or on a weekend, I will generally respond promptly the same day. If it is after hours or on a weekend, and it is not deemed an emergency, I will generally respond within 24-business hours.

***If you are having a medical or mental health emergency, you are advised to go to the nearest hospital emergency department or call 911 for emergency assistance. If in doubt, always seek emergency assistance. You may also call 988 for emergency crisis intervention. The is the national Suicide and Crisis Lifeline***

  • First/initial appointments are intake appointments. Much information is gathered via intake paperwork, screenings, and interview. Please allow a minimum of 60 minutes for the initial appointment. Pre-appointment completion of information is highly recommended and sent prior to the appointment via the portal.
  • Follow-up appointments are generally 30 minutes in length for medication management and 45 to 60 minutes in length for psychotherapy or counseling. If it is a combination medication management and psychotherapy appointment, please allow 60 minutes for the appointment.
  • When arriving for an appointment, whether online via telehealth or in-person, please allow 15-minutes past your scheduled appointment time to be seen as extra time may be needed by the appointment before you.
  • Please allow 3 business days for medication refills. Contact your pharmacy for all medication refills prescribed by me. If you do not receive your medication refill within 3 business days, please contact me directly as there may have been a transmission issue between the pharmacy and receipt by the provider.
  • ALL co-pays and self-pay payments are due at the beginning of the appointment and due in full unless other arrangements were made prior to the appointment. A debit/credit card is required to be kept on file for payments.
  • If you are requesting the Sliding Fee Discount Program, all documentation is to be completed in its entirety before acceptance into the program is permitted. This includes income verification(s).
  • Please contact provider either via phone or text message a minimum of 24-hours in advance of your scheduled appointment time if you need to cancel/reschedule your appointment or a one-time charge of $25 may be applied to your account for the missed appointment; see No Show, Late, and Cancellation Policy. Emergency exceptions may apply. 
  • If you are planning to apply for disability benefits, please know that a minimum of 6 months of care is required before documents are released to the requesting agency/yourself. This is necessary for the provider to be able to provide accurate/thorough information. I understand I may be charged a $100 minimum fee for the first 1-hour and $50 for each additional hour thereafter for any requested forms, letters, or other documents that are asked of the provider to complete outside of normal appointment time. I understand all such requests and paperwork should be completed during appointment time to ensure correct and adequate information is obtained and included. I understand I may request an appointment for paperwork requests/completion. I understand to allow my provider a minimum of 30-business days to complete all document requests to be completed. I understand the fees/charges are my responsibility. Exceptions may apply, which should be discussed with the provider.
